Innovative care is a specialising in the delivery of personal home care and nursing services for adults and children living with a Spinal Cord Injury, Acquired Brain Injury and other high care neurological condition.
Our Support Workers and Nurses receive specialised in-house training to ensure they have the skills and confidence to provide a high level of disability support services across Perth and WA. One of our Nurses will work with you to develop your unique care plan to address your personal care needs and provide individualised training to your care team.

Assistance with Social and Community Participation

Providing a range of home support services that vary from in-home assistance with household tasks like cleaning and gardening, right through to community support services and life skills development. We carefully match support workers with participants to support a better life and increased independence.
Innovative care supports participants to participate actively in the community, social and civic activities; we are committed to supporting you during these activities and strengthening participants’ ability to partake in these activities. We can also assist you in participating in group-based community, social, and recreational activities.
As a beneficiary of the NDIS program, participants can take part in a range of group and in-centre activities to improve their capability building and their overall wellbeing.
Transport and Public Activities
Innovative care can assist with Travel/Transportation arrangements for participants. This service includes: Building your capacity to independently travel by using public transport. Booking of taxi service Private transportation options Our staff members can drive you to all your daily activities such as appointments, community, social, work, school, or any other activities. It is our core responsibility to provide you a safe, reliable, suitable transport, scheduled to meet your needs.
These services include:
- School or another educational facility, work
- Your doctor or other medical appointments.
- Your local shopping centres.
- Around your community.
- All of our staff have a valid Driver’s License & Police check and valid qualifications to work in this Industry.
Psychosocial Recovery Coach
We offer personalised support and guidance to assist individuals with psychosocial challenges in achieving their recovery goals. We help clients navigate and implement their NDIS plan and enhance their overall well-being and ability to participate in daily activities…

Plan Management

Cleaning and Gardening
As a general rule, if you’re unable to maintain your garden on your own because of your disability, the gardening services are considered reasonable and necessary for you to live a normal life and relate to your NDIS goals, funding should be available. However, if a task requires a specialist, such as a landscape gardener to install a water feature, for instance, it won’t be covered by the NDIS, regardless of your disability.

Respite
A host family supports a person with disability in one of the most natural and beneficial ways possible: through connection, participation and a mutual sharing of lives.
Host families provide respite for carers by welcoming a person with disability into their family home on a full or part-time basis. In doing so, they gain valuable insight into the lives of people with disability, and reap the benefits of welcoming them into their own lives.

We begin with a comprehensive assessment which looks at all aspects of family life, and every family member who may potentially be involved. The application is then reviewed by an independent panel made up of experts in the field. A host family arrangement is then established by following a careful matching process, focusing on natural connections and relationship development.
To ensure the sustainability of the arrangement, our coordinator regularly visits the host family to reviews all aspects of the relationships and arranging any necessary training.
